Instructions
Complete the form below to check out the device.
Each authorized user will be given a badge that contains a QR code. Users may scan the QR code, which will bring the user to this page with the User ID prefilled. The Device ID is located on the device you are wanting to check out. Enter that Device ID into the field below and click 'Check out device'. If the device requires a passcode, the passcode will be displayed upon successful checkout. Follow the same process to check the device back in when completed. If you lose your badge, please contact an SYHA board member for assistance in completing the form.
